Another tool that you may want to check out is Survey Gizmo
(http://www.surveygizmo.com).  They have an accessibility section on their

(yeah - it is a bit of marketing hype)
 
I have not reviewed the entire system (just a few survey question types),
but they have some okay support for accessibility.  As with most systems, it
could be better.
 
A previous post mentioned WuFoo and that is another decent tool.  While both
could certainly improve the usability of these interfaces for use with AT,
they are not bad.
