Wufoo.com generates forms via a simple wizard, collects and hosts your data, and generates any reports or alerts you may need. Free, cheap or pricey depending on the features you need. See http://www.wufoo.com<http://www.wufoo.com/>
They assert that they work to keep the forms "as accessible as possible" though I'm not sure what that means; there are some sample forms you could test if you want to be sure. I suspect the real accessibility hiccups would come in the form creation tool itself, which is dependent on Ajax.

Subject: [Accessibility_sig] Registration form?
Can anyone provide me with information about creating registration forms that can be posted online? The Coalition of Texans with Disabilities just held the fourth annual Cinema Touching Disability film festival (www.ctdfilmfest.org<http://www.ctdfilmfest.org>), a film festival featuring movies about/starring people with disabilities. This year we included a competition for the best short disability film.
I would like to make it possible for filmmakers to register for the 2008 Cinema Touching Disability competition online. I do not, however know how to create a program to allow them to do so. Are there any "registration form" programs available online? I know that there are some programs, such as search engines, available online, and I was wondering if there are any "registration" programs available online.
I would appreciate any help /suggestions/comments.
